Personalization Form for Cakes & Custom Treats

Thank you for your interest in SouthPaw Treats!  We look forward to baking for your beloved pup!  To help make the process easier, please share the following details with us:

If ordering a BIRTHDAY or GOTCHA DAY CAKE:

1.  Style of cake (bone-shaped or 3-layer)

2. Message on cake (3-layer cake has limited space) 

3. Pup’s name- 

4. Requested decorations (flowers, paw prints, hearts, etc)

5. Preferred colors: (options- red, pink, orange, gold, green, blue, purple, black, white) 

6.  Date & Time requesting for order to be ready for pickup

 

If ordering a GENDER REVEAL CAKE:

1.  Who am I communicating with regarding this order? (parent-to-be, friend, etc.)  

2.  Message on cake (very limited space on a 4" cake)

3.  Pup's name

4.  Requested decorations (flowers, question marks, blue & pink bows, etc.)

5.  Preferred colors ( all white, white w/ yellow, white w/ another color, pink & blue, etc.)

6. Date & Time requesting for order to be ready for pickup

7.  Will the person picking up the order know the gender of the baby?

 

If ordering a custom treats:

1.  Would you prefer a themed set of treats or personalized bones? If a themed set, please describe.
